Unfortunately it happens quite a lot with older drivers getting mixed up when entering motorways.

Going by the available information, the accident happened around the Torre / San Miguel junction area, if as they say he had travelled 5klm, that means he got onto the wrong carriage way at the toll booths at La Zenia, how's that possible.
